Location
Foundations is located at 105 East Annandale Road, Suite 214, in Falls Church, Virginia.
There is abundant free parking available in our parking lot.

Paperwork
Once you have scheduled an appointment, Foundations will send you an extensive intake form. Please fill it out carefully and completely as it will help Foundations' therapists guide their initial evaluation and help them provide you the most efficient and comprehensive care. This paperwork also contains important consents which must be signed before treatment is rendered.
Payment
Method
Foundations requires you place a credit card on file in order to secure your appointment. Your credit card will be charged at the time of appointment. Foundations will accept cash at time of service, but is unable to accept checks at this time.
Chaperones
An important component of your healing is the development of the trusting, empathetic relationship between you and your therapist. However, Foundations recognizes that some clients may have had past experiences which make it difficult or uncomfortable to have one-on-one interactions with their therapist, especially during pelvic health care. A chaperone is not required, but Foundations welcomes their clients to bring a friend or family member with them to ensure all clients feel safe and comfortable as they are getting to know their treating therapist.
